We have reviewed whether or not to include Free PSA and the % Free PSA and have decided not to include them in the software because the interpretation of free PSA requires a user to evaluate Free PSA in the context of the total PSA reading. The software will only present straight numerical results and show whether or not that result fits within certain parameters. We cannot provide an interpretation of one value with a result of another value. Take this example, for instance:
The PSA Free has no biomarker range, which is problematic for the software, and the PSA, % free is 25 and considered low. The written text from the screenshot says, “The diagnostic usefulness of % Free PSA has not been established in patients with total PSA below 2.6 ng/mL.”
Unfortunately, we cannot currently convey this nuance in the reports we have in the software. This is something that you, the practitioner, will have to do with your patient. In the example above, I would tell the patient that the % free PSA result is not relevant because the PSA Total score is 0.4 ng/ml and the diagnostic usefulness of PSA % Free has not been established for PSA total levels below 2.6 ng/ml.
